Abstract: TRANSPLANTATION OF isolated islets of Langerhans provides a useful alternative to pancreas transplantation for the treatment of insulin dependent diabetes mellitus (IDDM). Although isolation techniques have improved, islet isolation still suffers from large variability in terms of islet yield and quality. A comprehensive quality control for isolated and purified islets should include assessment of islet quantity, histology, viability, and sterility. Currently applied quantitative viability tests are labor-intensive and time consuming. The following two established methods are commonly applied: (1) in vitro measurement of insulin release1; and (2) viability assessment by transplantation into immunodeficient rodents. The aim of this study was to test the reliability of high-resolution respirometry for functional assessment of rat islet isolations. In this system, cellular respiration is measured as an integral and sensitive signal reflecting ATP production. A respirometric approach for the analysis of mitochondrial capacity and membrane permeability should allow early recognition of cellular defects caused by islet processing. The method described can be performed rapidly and is therefore easily applicable in the clinical setting. We hypothesize, that analysis of membrane integrity and mitochondrial function by high-resolution respirometry may provide a guideline for further improvement of clinical islet transplantation.
